Request an Invoice

In order to complete a course registration, you may either pay by credit card or complete this invoicing procedure.

Please note that for the FTO and FTO Update courses we require 4 weeks advanced notice on cancellations due to the high demand and logistical concerns associated with that training. All others require 2 weeks advanced notice for a full refund. Please read our full cancellation policy here. By submitting the form below, you acknowledge that you have read and are in agreement with this policy and that no exceptions may be made.

If you would like to pay by invoice, please fill out the form below. If you don’t yet know the student’s names yet, just list them as “To Be Determined 1, To Be Determined 2, etc.” When we receive the information, we’ll verify there is room in the course and then send you a confirmation with a note indicating “To be invoiced”. Usually this will occur within the same business day (M – F). You’ll receive an invoice shortly thereafter.

While we’re happy to send you an invoice, please be advised there is an additional $15.00 per invoice, non-refundable processing fee.
